It is necessary to understand the anatomy of the retroperitoneal spaces including the pancreas and mesentery to diagnose pancreatic diseases correctly. Though non ─ contrast CT is adequate to diagnose acute pancreatitis, contrast enhanced C … T is essential for examination of the severity and complication of acute pancreatitis. Occasionally, invasion of the main pancreatic duct by pancreatic carcinoma may cause acute pancreatitis. Contrast ─ enhanced CT is recommended to detect small pancreatic cancers, because non ─ contrast CT cannot detect these tumors. Magnetic resonance imaging(MRI) and MR cholangiopancreatography (MRCP)are able to clearly depict biliary stones, hemorrhagic fat necrosis, and hemorrhage within the pseudocysts associated with acute pancreatitis. MRCP also illustrates both bile and pancreatic ducts without contrast medium, and is suitable for screening for the etiologies of pancreatitis such as an anomalous connection between the bile and pancreatic ducts. 膵疾患を正確に診断するためには，後腹膜腔ならびに膵と腸管（大腸，小腸）を結ぶ間膜（結腸間膜，腸間膜）の解剖学的理解が必要である。単純CTでも急性膵炎の診断は十分可能であるが，重症度や合併症の評価には造影 CTが不可欠である。また，時に膵癌が急性膵炎の原因となることがあるが，単純 CTのみでは膵癌の検出は困難なことが多く，造影ダイナミックCTが必要である。MRI（以下，MRCP）は胆道結石の診断能が高く，急性膵炎に随伴する出血性脂肪壊死，仮性.胞内出血などの出血性変化の評価にも有用性が高い。MRCPは造影剤を使用することなく，胆管膵管の全体像を描出できるので，膵炎の原因精査（胆管膵管奇型）にも推奨される。 続きを見る
