A rapid increase in penicillin resistance (Per) to Streptococcus pneumoniae (S. pneumoniae)and b lactamase-negative ampicillin-resistant strains of Heamophilus Influenzae (BLNAR) have recently been reported in Japan. Per to S. pneumoniae an … d BLNAR is the principal cause of recurrent AOM. In 2006, the Japanese Society of Otorhinolaryngology Subcommittee on the Management of Acute Otitis Media published “The Guidelines for the Diagnosis and Management of Acute Otitis Media”. We investigated how many clinicians use these Guidelines and their evaluation of them. We received 149 responses from Otolaryngologists (n= 55) and Pediatricians (n=94) in Ishikawa Prefecture. Fifty-two percent of Pediatricians and 85% of Otolaryngologists were aware of these Guidelines and about half have used the Guidelines. About 60% of all doctors think that the Guidelines are worthy of practical use. 急性中耳炎起炎菌の薬剤耐性株の市中における蔓延にともない,抗菌化学療法の限界を示すような難治性の急性中耳炎症例が増加している状況において,2006年に日本耳鼻咽喉科学会関連3学会,研究会から「小児急性中耳炎診療ガイドライン」が提唱された. 今回小児の急性中耳炎診療にたずさわる耳鼻咽喉科医・小児科医を対象に,本ガイドラインがどの程度認知され,実際に活用されているかを知るためにアンケート調査を行った. その結果,耳鼻咽喉科医の85%がガイドラインの存在を知っており,耳鼻咽喉科医への認知度はある程度進んでいた. しかし耳鼻咽喉科医・小児科医ともに全体の40%程度の医師は今後もガイドラインを参考にする必要を感じていないという結果であり,特にクリニック開業医師にその傾向が強かった. ガイドラインは,あくまでも診療にあたって臨床的判断を支援するためのものである点を周知し,さらなる普及が必要である. 続きを見る
