概要: A case of intraductal papillary neoplasm of the bile duct (IPNB) arising in a patient with hepatitis B-related liver cir rhosis with hepatocellular carcinoma (HCC) is reported. A 76-year-old man was admitted to our hospital with recurrent HCC. Laboratory data showed that levels of carcinoembryonic antigen and carbohydrate antigen 19-9 were elevated. He died of progressive hepatic failure. At autopsy, in addition to HCCs, an intraductal papillary proliferation of malignant cholan-giocytes with fibrovascular cores was found in the dilated large bile ducts in the left lobe, and this papillary carcinoma was associated with an invasive mucinous carcinoma (invasive IPNB). Interestingly, extensive intraductal spread of the cholangiocarcinoma was found from the reactive bile ductular level to the interlobular bile ducts and septal bile ducts and to the large bile ducts in the left lobe. Neural cell adhesion molecule, a hepatic progenitor cell marker, was detected in IPNB cells. It seems possible in this case that hepatic progenitor cells located in reactive bile ductules in liver cirrhosis may have been responsible for the development of the cholangiocarcinoma and HCC, and that the former could have spread in the intrahepatic bile ducts and eventually formed grossly visible IPNB. © 2011 Baishideng. 概要: 金沢大学医薬保健研究域保健学系<br />Recent studies have clarified that endogenous cannabinoids (endocannabinoids) are released from dep olarized postsynaptic neurons in a Ca2+-dependent manner and act retrogradely on presynaptic cannabinoid receptors to suppress inhibitory or excitatory neurotransmitter release. This type of modulation has been found in the hippocampus and cerebellum and was called depolarization-induced suppression of inhibition (DSI) or excitation (DSE). In this study, we quantitatively examined the effects of postsynaptic depolarization and a cannabinoid agonist on excitatory and inhibitory synapses in rat hippocampal slices and cultures. We found that both DSE and DSI can be induced, but DSE was much less prominent than DSI. For the induction of DSE, the necessary duration of depolarization was longer than for DSI. The magnitude of DSE was much smaller than that of DSI. To explore the reasons for these differences, we tested the sensitivity of EPSCs and IPSCs to a cannabinoid agonist, WIN55,212-2, in hippocampal cultures. IPSCs were dichotomized into two distinct populations, one with a high sensitivity to WIN55,212-2 (50% block at 2 nM) and the other with no sensitivity. In contrast, EPSCs were homogeneous and exhibited a low sensitivity to WIN55,212-2 (50% block at 60 nM). We estimated that the 5 sec depolarization elevated the local endocannabinoid concentration to a level equivalent to several nanomoles of WIN55,212-2. Using CB1 knock-out mice, we verified that both DSI and DSE were mediated by the cannabinoid CB1 receptor. These results indicate that presynaptic cannabinoid sensitivity is a major factor that determines the extent of DSI and DSE. 続きを見る

概要: 金沢大学附属病院放射線科<br />肝幹細胞の形質を有し、予後不良なEpCAM陽性肝細胞癌ではダイナミックCT動脈相における不均一な濃染パターン、内部の造影不染域含有、MRIの拡散強調像におけるより低値なADC値、Gd-EOB-DTPA造影 MRI(EOB-MRI)肝細胞相におけるnon-smooth marginという画像特徴をEpCAM陰性肝細胞癌群に比較して有意に高い頻度で認められた。これらの画像所見はEpCAM陽性肝細胞癌を推察するのに有用であり、将来的な個別化医療に貢献するものと考えられる。一方、EMT関連HCCを画像で推察することは難しいと考えられたが、EOB-MRIの肝細胞相での増強率がより低値を示す傾向を示した。<br />Epithelial cell adhesion molecule (EpCAM) positive hepatocellular carcinomas (HCC) having hepatic stem cell feature showed significantly higher incidence of imaging features including a heterogeneous arterial enhancement and internal no enhancement portion on dynamic CT study, lower apparent diffusion coefficient (ADC) value on diffusion weighted image, and non-smooth margin on hepatobiliary phase (HB phse) of Gd-EOB-DTPA enhanced MRI (EOB-MRI). These imaging findings are useful for predicting EpCAM positive hepatocellular carcinoma and are thought to contribute to future personalized medicine.Regarding epithelial-mesenchymal transition (EMT) positive HCCs, there was no significant imaging features that can predict epithelial-mesenchymal transition (EMT) positive HCCs.
