概要: 金沢大学保健管理センター<br />115<br />To clarify the long-term efficacy, safety and the cytokine network-modulating effects of tacr olimus in myasthenia gravis, medical records of 86 newly diagnosed consecutive patients and nine steroid-dependent patients were retrospectively reviewed, and peripheral blood mononuclear cells (PBMC) were cultured for the cytokine profile. Steroid reduction effects were observed by using tacrolimus, and no serious adverse effects were observed. The culture study showed reduced IL-12, IL-17, IFN-γ, GM-CSF, TNF-α and MIP-1β, and elevated IL-10 in the PBMC from patients who received tacrolimus, which suggests inhibition of T cells and macrophages, and enhancement of type 1 regulatory T cells. © 2008 Elsevier B.V. 概要: Objective: The purpose of this study is to investigate the usefulness of sympathetic skin response (SSR) and heart rate variability (HRV) for the differential diagnosis of patients with dementia with Lewy bodies (DLB). Design: A diagnostic test study. Setting: Single centre in Japan. Participants: We examined 20 patients with probable Alzheimer's disease (AD) diagnosed with NINCDSADRDA criteria and 20 with probable DLB diagnosed with the criteria of the third international DLB workshop. Methods: For the SSR measurement, surface electrodes were used: the active recording electrode was placed on the palm of the hand and the reference electrode was placed on the dorsum of the same hand. SSR was induced by a median nerve electrical stimulation at an amplitude of 20 mA. For the HRV measurement, the A-A intervals were measured twice for 2 min with an interval of 5 min in a sitting position after a rest of 5 min. From the low-frequency power (LF; 0.02-0.15 Hz) and high-frequency power (HF; 0.15-0.50 Hz), the ratio of LF to HF power (LF/HF) was calculated using the maximal entropy method. Results: SSR and HRV could detect the abnormality of autonomic function in patients with DLB at sensitivities of 85% and 90%, respectively. On the other hand, SSR and HRV detected an abnormality of autonomic function in patients with AD at sensitivities of 15% and 25% (p<0.05). The combination of the SSR and the HRV (double-positive) indicated abnormal autonomic function was recorded in only 1 of 20 patients (5%) with AD. In contrast, this combination indicated autonomic abnormality in 15 of 20 patients with DLB by our criteria (75%). Conclusions: SSR and HRV can be applied to differentiate DLB from AD. 続きを見る

概要: Autoantibody against nicotinic acetylcholine receptor (nAChR) α3 subunit has been implicated in the pathogenesis of para neoplastic neurological syndrome. To examine the effect of anti-α3 subunit autoantibody on cell-surface nAChRs, we established human embryonic kidney 293 cells stably co-expressing α3 and β4 subunits. Upon incubation with seropositive patient's serum, this cell line showed co-accumulation of patient's IgG and α3 subunits in the cytoplasm. These data support the hypothesis that anti-α3 subunit autoantibody induces internalization of cell-surface nAChRs and thereby impairs synaptic transmission. © 2012 Elsevier B.V. 概要: Objectives: To evaluate the ability of tacrolimus to reduce the corticosteroid dose in patients with myasthenia gravis (MG) and the drug's safety in a double-blind, placebo-controlled, parallel group study. Methods: Patients being treated with oral prednisolone at doses equivalent to 10-20 mg/day, and with stable symptoms, were randomised to tacrolimus or placebo in a 28-week double-blind study. The dose of corticosteroid was tapered with the procedures specified in the protocol. The primary efficacy endpoint was the mean daily prednisolone dose given in the last 12 weeks of the study. Results: Eighty patients received the study drug (40 patients in each group) and were included in the full analysis set. In the full analysis set, there was no significant difference in the primary efficacy endpoint between the two groups (p = 0.078). However, some secondary analyses suggested the steroid-sparing effect of tacrolimus. Tacrolimus was well tolerated, and no safety concerns were noted. Conclusions: This study suggests that tacrolimus has a potential advantage as a steroid-sparing agent in the treatment of MG patients. Copyright Article author (or their employer) 2011. 続きを見る

Using this method, we detected DHPR antibody in 37% (11 out of 30) of MG patients with thymoma. Antibodies were not detected in normal nor disease controls. The titer of DHPR antibodies showed no significant correlation with autoantibodies to acetylcholine nor ryanodine receptors. 概要: 金沢大学医薬保健研究域医学系<br />TE671 is a cell line obtained from the rhabdomyosarcoma cells. In the present study, we examined [Ca 2+]i alteration induced by acetylcholine (ACh) in TE671/RD cells with the special attention to ryanodine receptor (RyR) and inositol 1,4,5-triphosphate receptor (IP3R). The change of [Ca2+]i was mediated by muscarinic type 3 (m3) AChR. Both phosphatidylinositol-specific phospholipase C blocker (U73122) and IP3R blocker (2-APB) inhibited ACh-induced [Ca2+]i elevation, suggesting that IP3 pathway is involved in [Ca2+]i alteration. Ryanodine and FK506 increased ACh-induced [Ca2+]i elevation, which was decreased by RyR blocker (ruthenium red). These results suggest that RyR and FK506 binding protein 12 kDa (FKBP12) complex was involved in [Ca2+]i change, and that TE671/RD cell line has a hybrid characteristic of smooth and skeletal muscles.
