概要: Aim of this study was to assess the relation of generalized obesity and fat distribution to sympathetic and parasympathetic components ofheart rate in healthy young subjects. We recorded ECG continuously in the supine and standing position with pacing respiration at 0.25Hz for 5 min in 47 healthy young female. Habitus was represented by body mass index, as an index of generalized adiposity, and by the ratio of waist-to-hip girth (waist/hip ratio), as an index of centripetal fat distribution. A spectral analysis of R-R variability from 256-sec series interbeat interval in each position provided markers of sympathetic nerve activity (ratio of low frequency component to high frequency component (LF /HF ratio) in the standing position), and vagal activity (HF power in the supine position). LF /HF ratio in the standing position was positively correlated with both waist/ hip ratio (r=0.366, p<0.02) and waist girth (r= 0.317, p<0.04), but not correlated with body mass index (p=0.904). HF power in the supine position was not correlated with waist/hip ratio (p=0.893), waist girth (p= 0.907) or body .mass index (p=0.454). These results suggest that, in healthy young female, centripetal fat distribution is one of the determinants of the cardiac sympathetic activity, but not the cardiac vagal activity. 続きを見る

概要: 金沢大学医薬保健研究域医学系<br />Aims: The lysophospholipid mediator sphingosine-1-phosphate (S1P) activates G protein-coupled recept ors (GPCRs) to induce potent inhibition of platelet-derived growth factor (PDGF)-induced Rac activation and, thereby, chemotaxis in rat vascular smooth muscle cells (VSMCs). We explored the heterotrimeric G protein and the downstream mechanism that mediated S1P inhibition of Rac and cell migration in VSMCs. Methods and results: S1P inhibition of PDGF-induced cell migration and Rac activation in VSMCs was abolished by the selective S1P2 receptor antagonist JTE-013. The C-terminal peptides of Gα subunits (Gα-CTs) act as specific inhibitors of respective G protein-GPCR coupling. Adenovirus-mediated expression of Gα12-CT, Gα13-CT, and Gα q-CT, but not that of Gαs-CT or LacZ or pertussis toxin treatment, abrogated S1P inhibition of PDGF-induced Rac activation and migration, indicating that both G12/13 and Gq classes are necessary for the S1P inhibition. The expression of Gαq-CT as well as Gα12-CT and Gα13-CT also abolished S1P-induced Rho stimulation. C3 toxin, but not a Rho kinase inhibitor or a dominant negative form of Rho kinase, abolished S1P inhibition of PDGF-induced Rac activation and cell migration. The angiotensin II receptor AT1, which robustly couples to Gq, did not mediate either Rho activation or inhibition of PDGF-induced Rac activation or migration, suggesting that activation of Gq alone was not sufficient for Rho activation and resultant Rac inhibition. However, the AT1 receptor fused to Gα12 was able to induce not only Rho stimulation but also inhibition of PDGF-induced Rac activation and migration. Phospholipase C inhibition did not affect S1P-induced Rho activation, and protein kinase C activation by a phorbol ester did not mimic S1P action, suggesting that S1P inhibition of migration or Rac was not dependent on the phospholipase C pathway. Conclusion: These observations together suggest that S1P2 mediates inhibition of Rac and migration through the coordinated action of G 12/13 and Gq for Rho activation in VSMCs. © The Author 2008.. 続きを見る

概要: Background: Intestinal angina is characterized by recurrent postprandial abdominal pain and anorexia. Commonly, these sy mptoms are caused by severe stenosis of at least two vessels among the celiac and mesenteric arteries. However, intestinal perfusion is affected not only by the degree of arterial stenosis but also by systemic perfusion. We experienced a unique case of intestinal angina caused by relatively mild stenosis of the abdominal arteries complicated with hypertrophic obstructive cardiomyopathy. Case presentation: We report an 86-year old Japanese man with hypertrophic obstructive cardiomyopathy and advanced atrioventricular block who was diagnosed with intestinal angina. Computed tomography showed mild stenosis of the celiac artery and severe stenosis of the inferior mesenteric artery, and these lesions were relatively mild compared with other reports. A dual-chamber pacemaker with right ventricular apical pacing was implanted to improve the obstruction of the left ventricular outflow tract. After implantation, the patient's abdominal symptoms diminished markedly, and improvement of the left ventricular outflow tract obstruction was observed. Conclusions: Although intestinal angina is generally defined by severe stenosis of at least two vessels among the celiac and mesenteric arteries, the present case suggests that hemodynamic changes can greatly affect intestinal perfusion and induce intestinal angina in the presence of mild stenosis of the celiac and mesenteric arteries. © 2016 The Author(s). 続きを見る

概要: 金沢大学<br />本研究は,無拘束(携帯型)循環動態連続計測・自律神経活動解析システムの開発に関する発展的研究であり,計測原理として,血圧計測に容積補償法,心拍出量計測に電気的アドミタンス(インピーダンス)法を用い,非侵襲的に一心拍毎の血 圧・心拍出量を含む循環動態計測が可能となるシステム開発を目指したものである.また,これら循環動態情報は,自律神経系の支配を受けているため,時系列あるいは時間周波数解析を行うことで,自律神経活動の評価・解析も可能となる統合システムを目指して研究開発が行われた. しかし,連続血圧計測において,血管壁の物理特性や組織内血液量などが計測中に大きく変化する際に誤差を生ずることや,心拍出量計測において,血液量変化が実際には,肺循環などでも起きているが,大動脈のみで起きていると仮定しているため,相対変化としての追従性は良いが,絶対値としての信頼性が低いことが判明している.本研究では,これら誤差要因を解明し,さらなる用途拡大を目指すこととした. 心拍出量計測においては,胸郭体表面等電位分布計測システム(胸郭全体計測に対しては32点(8×4)×2(胸部前・背面)および胸部前面正中近傍計測に対しては,24点(4×6)のスポット電極で構成される胸部インピーダンスマッピングシステム)を開発した.本システムを用い,電極周囲で電流集中が発生せず,姿勢変化の影響を受けにくい電極配置として,電流通電電極については,一方の電極を頭部の耳の後ろ付近,他方を下腹部(腰骨付近)とすること,また,検出電極については,肺循環などの影響を受けにくい鎖骨中央と剣状突起間に電極を配置することの妥当性が確認された.これらの結果より電極のスポット電極化ならびに,計測精度も向上することも確認された. 血圧計測においては,容積補償法の制御目標値に追従することを主眼においた制御法の開発,および鬱血を回避可能な局所加圧法の開発を通し,血圧曲線は直接法と比較して計測精度の向上が確認された. 自律神経活動評価においては,平均血圧値と心拍出量から得られる総末梢循環抵抗などを用いることで,心電図解析では得られない交感神経系単独の活動を評価することが可能となった. 本研究を通し,プロトタイプシステムの開発に成功し,今後さらにシステムの実用化開発と,特に生体ストレス評価に本システムを適用した応用研究を展開していく予定である.<br />In order to assess on-line autonomic regulation of cardiovascular system during daily activities, a non-invasive and ambulatory system has been successfully developed, which can automatically monitor instantaneous digital or radial systolic (SBP), mean (MBP), and diastolic blood pressure (DBP), stroke volume (SV), heart rate (HR), cardiac output (CO=SV・HR), and total peripheral circulatory resistance (TPR=MBP/CO). Principle of the instantaneous blood pressure measurement is based on the volume-compensation method using the photoplethysmographic vascular unloading technique with a newly designed partially pressurization cuff, while that of the CO measurement based on the electrical admittance (or impedance) cardiography with newly proposed spot electrodes alignment. As to accuracy of the system, the performance of the instantaneous radial blood pressure measurement method was evaluated with the direct blood pressure measurement using 9 healthy subjects. The results of the Bland-Altman an alysis of the SBP and DBP measurements were -0.46±4.1 and 0.63±3.3mmHg (mean±1.96SD, (accuracy±precision)), respectively. The performances of the admittance cardiography with the spot electrodes (the current source electrode pair : behind of the ears-right side of the waist, the detection electrode pair : manubrium-xiphoid process (the both ends of the sternum)) were also evaluated with the dye-dilution cardiac output measurement. Appling to the Kubicek's method of the impedance (or admittance) cardiography, the assumption of the homogeneity of the current distribution of the thorax between these spat electrodes was confirmed by the 24 (4 x 6) spot electrodes thorax impedance mapping system. The result of the Bland-Altman analysis of the CO measurements was 0.2±2.0l/min (mean±1.96SD). Using this system, the autonomic regulation analysis was conducted by the spectral method of the FFT and MEM (Maximum Entropy Method). The high frequency component (HF : 0.15-0.5Hz) of the IBI (inter-beat interval) has been recognized as the index of the vagal nerve activity. The low frequency component (LF : 0.04-0.12Hz) of the TPR was proposed as the solely index of the sympathetic nerve activity. However, these indexes had been failed to describe sudden changes of the human activities due to long range of data requirements, i.e. 64-128 points.
