(ELISA system), using Markit-M Excegran and Markit-M Haloperidol (Dainippon Pharmaceutical Co.) was evaluated to determine the concentrations of zonisamide (ZNS) and haloperidol (HP) in human plasma. At first, the within-run (ZNS : n=8, HP : n=10) and the between-run (4 days) precisions of ZNS and HP were examined. The coefficient of the variations in both the within-run and the between-run previsions of HP were below 5.0 and 7.00/0, respectively. In contrast, the some values of ZNS were below 6.5 and 8.40/0, respectively. A good correlation was observed between this ELISA system and HPLC (r=0.917), which was used to determine the plasma ZNS concentration. In the case of HP, a good correlation was seen between this ELISA system and the manual ELISA method (r=0.978). This method was thus used to monitor the serum levels in patients receiving ZNS or HP therapy. The relationship was observed between the daily dose (mg/kg) and the plasma concentration of the trough level for ZNS and HP, respectively (r=0.750 p<0.001, r= 0.669 p<0.001). The plasma concentration at 24hr after i. v. injection of 3.2 mg/kg varied between 0.4 ng/ml (10-week-old fasted rats) and 1.4 ng/ml (10-week-old normal rats). The steady state distribution volume of biperiden in the fasted rats was smaller by 2/3 than that in the normal rats. The determined percentage of fat tissue per body weight reduced from 7.6 to 2.35 was caused by fasting. There was a good cerrelation between the steady state distribution volume of biperiden per lean mass body weight and the fat volume per lean mass body weight (r=0.989) in four different groups (10-week-old fasted 4-, 10-, and 50-week-old normal rats). The fat/plasma concentration ratios at 8hr after the i. v. injection varied between 380 (10-week-old normal rats) and 550 (10-week-old fasted rats), whereas the brain/plasma concentration ratios were identical to those at steady state among the two groups. The time courses of biperiden concentration in plasma, brain, and fat were simulated using a physiological pharmacokinetic model. There was reasonable agreement between the model predictions and the observed data, suggesting that the change in the fat volume is a dominant determinant of the distribution volume of biperiden in rats. Changes in fasted tissue and plasma concentrations are discussed in relation to the clinical usefulness of the blood level monitoring. 続きを見る

The assay employs the partially purified synaptosomal membrane isolated from rat brain. In the assay, 3H-Quinuclidinyl benzylate competes with drugs for binding to the receptor. The detection limit for scopolamine was 20 ng/ml. This method was applied for the measurement of other drugs acting on the central nervous system. The values of IC 50 were lower than 0.1μg/ml in biperiden, trihexyphenidyl, levomepromazine, and promethazine. The method has been used to determine plasma levels in 9-week-old and 21-week-old rats after i.v. injection of 1 mg/kg scopolamine. The blood levels of scopolamine at each time after the administration are significantly different between 9-week-old and 21-week-old rats. The values of V_<dss> determined in 21-week-old rats were significantly increased as compared to the 9-week-old rats, whereas CL_<tot> were not changed. 続きを見る

概要: 金沢大学附属病院薬剤部<br />Drug information supplying system by using computer (ACOS-630 model 10:NEC) named ゛On-line Hospital For mulary System" was newly developed in Kanazawa University Hospital. The special features of this system are 1) consisting of two types of datafiles, 2) three ways of retrieval including our original therapeutic code retrieval, 3) easy operation, and 4) short time required for maintenance of datafiles. The two types of datafiles are 1) basic information (e.g., dosage forms, adverse effects, precautions for administration) about the available drugs in our hospital and 2) the useful information for effective drug therapy made up by pharmacy staffs. Both datafiles are available to all medical and nursing staffs via computer terminals located in each nursing station and medical office. The maintenance of this system is well performed by pharmacy staffs. Data input and datafile maintenance are performed by on-line word processing software system, and time required for them is about six hours per month. 続きを見る

概要: 金沢大学附属病院薬剤部<br />To reduce the working hours and manpower to edit the bound hospital formulary (bound HF), we tried to m ake the editing manuscript by using the datafiles of ""on-line hospital formulary system (on-line HFS)"" operating in Kanazawa University Hospital which was developed by our pharmacy staffs in October 1987. And the printing form of the new bound HF was changed to be same with the display image of the ""on-line HFS"" to use the two systems conveniently. Because the data of ""on-line HFS"" is revised continually, the editing manuscript formed in floppy disk was prepared simply from ""on-line HFS"" for only 30 min and sent to the press. The works to write the manuscript and to correct proofs were considerably reduced by this way. Consequently, the time to publish the new bound HF which was 99 days was reduced as compared with 235 days for the last publishing. 続きを見る

概要: 金沢大学医学部付属病院薬剤部<br />University Medical Information Network (UMIN) has been developed by connecting all of the national u niversity hospital host computers. Through this network, hospital personnel share in the benefits of various medical information services. We attempted to send/receive MSDOS drug information system files by UMIN. UMIN does not accept binary data, but the "ish file converter" has enabled us to convert binary data files to text files for uploading and downloading. New ADMICS (Advanced Drug Mixture Information & Consultation System) has been developed on FMR (Fujitsu) personal computers by investigating ADMICS files on PC-9800 (NEC) personal computers. A batch file made it possible to update ADMICS automatically and immediately with files downloaded from UMIN. UMIN is expected to facilitate collaboration among university hospitals in utilizing application programs and databases originally developed on MS-DOS at each university hospital. 続きを見る

概要: 金沢大学医学部付属病院<br />The supply of injectable drugs for individual patients using the prescription order entry system was st arted from Apr. of 1995 at Kanazawa University Hospital. The injectable drugs for individual patient are set up by pharmacists into each compartment of the kart (Ordering Kart : Sakase Corporation), and then are carried by nursing assistants to the respective clinical areas. The characteristics of this supply method are [1] it is set up by 4 pharmacists on a daily schedule (8 : 30 am-3 : 30 pm) . [2] injectable drugs for emergency are louded in a special compartment of the carrying kart, and [3] are set up for prescription entry by the physicians within 24 hours. As a result, it is thus possible to check the prescription (dosage, route, incompatibility and stability), to reduce the stock of injectable drugs in the ward while, in addition, manpower for both the nursing and medical office personnel can be reduced. 続きを見る

概要: 金沢大学医学部付属病院薬剤部<br />The prescription entry system for injectable drugs which runs on the ACOS system 3600/6 (ORDERIG : N EC Corporation) was newly implemented on Apr.3 1995 for both inpatients and outpatients of Kanazawa University Hospital. The system has been accepted by both physicians and nurses. A 100% entried prescription rate via the system was achieved within one week after the implementation and this system has continued to run smoothly without any problems up to, now. The implementation of thus system made the supply of injectable drugs for individual patients possible, but the system still needs to develop a check fuction for prescriptions (especialy regarding imcompatibility). 続きを見る
