問紙調査を行った。フットケアの実施の実態と課題について単純集計を行い、実施機関別に検討した。【結果】275 センターから回答があった（回収率 31.2％）。回答のあった施設の内訳は、直営型地域包括支援センター（以下直営型）53 センター（19.3％）、委託型地域包括支援センター（以下委託型）219 センター（79.9％）であった。フットケアを実施した経験があったのは、59 センター（21.5％）であった。フットケアの位置づけは、転倒防止が 36 センター（61.0％）と最も多かった。方法は、知識と演習の組み合わせが 52 センター（88.1％）であり、演習内容は足指体操が 40 センター（67.8％）、足のマッサージ 39 センター（66.1％）の順に多かった。直営型は委託型に比べ、月 1 回以上実施している割合が有意に多かった（P<0.001）。フットケアを行っていないセンターにおいて必要な体制は、フットケアの指導ができる専門職のサポートが 136 センター（63.0％）と最も多かった。【考察】フットケアを事業として実施したことのある地域包括支援センターは、回答のあったセンターのうち約 2 割であり、実際はさらに少ない割合であると考えられた。現状ではフットケアを実施するスタッフをセンター内で全てを確保することは難しい。地域の中で専門職と連携し、専門職がフットケア事業をサポート出来るような体制作りが必要であると示唆が得られた。<br />This study was performed to clarify the implementation status and issues associated with foot care in preventive long-term care.We searched the Internet for community comprehensive support centers where foot care information was being provided to elderly people, and found such centers in seven prefectures. A survey of 882 facilities was conducted by sending self-administered questionnaires by postal mail. We compiled simple statistics on the current conditions and issues associated with foot care implementation and analyzed data according to community comprehensive support center type.We received responses from 275 centers （31.2%）. The facilities were classified as follows: directly operated city or town community comprehensive support centers （direct-managed type）, n = 53 （19.3%）, and outsourced community comprehensive support centers （entrusted type）, n = 219 （79.9%）.Fifty-nine centers had experience with foot care （21.5%）. The most frequent objective for foot care was to prevent falls （n = 36 centers, 61.0%）. Fifty-two centers （88.1%） used both advice and practical foot care exercises. These exercises consisted of foot care and toe movement （n = 40 centers, 67.8%） and foot massage （n = 39 centers, 66.1%）. A significantly higher percentage of directly managed type centers offered foot care more than once a month （P < 0.001）. Of the centers that did not provide foot care, 136 centers responded the requirement for a professional support system for foot care guidance as the reason （63.0%）. The community comprehensive support centers that had implemented foot care accounted for about 20% of the centers that responded, which we consider to be an extremely low proportion. It is difficult to secure all of the necessary staff to implement foot care from only community comprehensive support centers. Thus, it is necessary to cooperate with professionals in communities and to establish a framework in which support can be provided by foot care professionals for foot care projects.

合が有意に低下した（ｐ＜0.01）。過去6か月間の転倒歴は、介入群の１名は介入後０名に減少し、対照群は1名から介入後４名に増加した。3年目以降の調査は介入群のみとし、介入6か月後と1年後を評価した。足の手入れは平均週2回以上であった。ＢＭＩは1年後有意に低下し（Ｐ＜0.05）、ファンクショナルリーチは継続に伴い改善がみられた。バランスの安定感や接地感覚は、ほぼ全員があると評価した。<br />We created a foot care program for elderly who were physically independent, and assessed physical and mental function after 6 months for both the control group and intervention group. Beginning at the end of 2013. In the intervention group, after 6 months there was significant improvement in foot form. In the control group, there was no significant improvement (p<0.01). There were no falls in the intervention group, however there were four falls in the control groupA survey was conducted with only the intervention group, and subjects were evaluated 6 months, and one year after intervention. The average frequency of foot care was more than twice a week. BMI decreased significantly after 1 year (p<0.05). Functional reach improved with continuation of self-care. 歩数には年齢,介護予防評価,BMIが有意に関連した。冬期の間に左の足指力は有意に低下した。冬期に比べ秋期の平均歩数は有意に多かった。降雪地域の冬期の活動量維持プログラムとして,認知機能の維持と足の機能保持を取り入れることへの示唆が得られた。<br />In this study we evaluated physical and mental conditions of the elderly in winter, after previously looking at them in autumn, identifying issues and devising a possible long term care prevent program. Parameters evaluated for the elderly included were the average amount of activity, age, cognitive function, care prevention evaluation, average room temperature during the day, and BMI. There was a significant correlation between BMI and physical activity as measured by the number of steps taken per day. Power in the left leg decreased significantly during winter. The average number of steps in autumn was significantly higher than winter.
